What is Bi-est Cream?

Bi-est cream is a product containing two estrogens, estradiol and estriol.

These hormones are natural hormones that are identical to the hormones found naturally in the body.  Hence the name bio-identical.

This cream is often a part of a bio-identical hormone replacement therapy plan for women.  It is used to treat perimenopausal, menopausal and hormone imbalance symptoms.  It may also have anti-aging effects.

The cream is made by combining estradiol (E2) as well as estriol (E3) and can be obtained at our compounding pharmacy with a doctor’s prescription.

For women who have already reached menopause, the dominant estrogen types that are produced by the ovaries include estradiol and estrone.

Another estrogen that is produced is estriol and is considered the safest of the three. As such, bi-est cream will have estriol to balance the effects of estradiol.

In perimenopausal and menopausal women, the low levels of estrogen result in several symptoms that range from sleep disturbances, irregular periods, dermatology issues like acne, vaginal dryness, hair thinning, and excess sweating. After some time, low estrogen levels can result in more severe health complications like heart disease, depression, and osteoporosis.

This is the main reason why bioidentical hormone replacement therapy programs and services are essential.  These services help women who are already showing signs of low estrogen levels.

Progesterone is also often added to the therapy if saliva test show that progesterone levels are low.

Bi-est cream is, therefore, a compounded cream that contains two estrogens most commonly in concentrations of 80% estriol and 20% estradiol.  Other concentrations may also be used.

Bi-est may also be used together with progesterone if it is determined to be necessary.

What is bi-est topical used for?

Throughout a woman’s reproductive years, the ovaries produce hormones naturally that balance the body and are responsible for functions such as regular menstrual periods and an overall healthy reproductive system.

As a woman ages, and enters the menopause phase, hormones such as estrogen are not produced in enough quantities to maintain this balance which leads to the common menopausal symptoms like vaginal dryness, irregular periods, hot flashes, slow metabolism, night sweats, moods changes among others.

The aesthetics of hormone replacement therapy aim at replacing these hormones and the best care and solution is the use of meds such as bi-est cream.

Through hormone replacement therapy (HRT), menopausal symptoms are reduced or eliminated and work the same way as testosterone replacement therapy in men.

How does bi-est cream work?

If you are a woman who has already reached the age of menopause, you may already have started experiencing symptoms like vaginal dryness and irregular periods. Such symptoms are caused by low estrogen levels in the body.

As such your doctor may prescribe HRT as one of the solutions to treat the symptoms and reduce other age-related brain issues like memory loss.

Bioidentical hormones like Bi-est cream, in general, have a chemical composition that is similar to the hormones that are produced by the ovaries.

When taken orally, these bioidentical hormones like the ones that come as capsules are converted in the liver.

However, to get the most of the therapeutic benefits of bioidentical hormones, the topical form is the best form of treatment since the doctor will customize the dose to meet the different needs of menopausal women.

What are the potential side effects of Bi-est topical?

Like with every other medicine that is prescribed to patients, there are some potential side effects to using bi-est cream that you should look out for.

Such side effects include vaginal bleeding, bloating, nausea or vomiting, breast tenderness and maybe pain, changes in appetite, fluid retention, weight gain, and in some people irritability and anxiety some of which are similar to hypothyroidism that results due to low thyroid hormone production.

These potential adverse effects vary from one patient to another.  It is important you communicate with your doctor or pharmacist about what you are experiencing.

Most symptoms are however temporary and may appear when you first start using the medication.   Most of them will resolve as your body adjusts to the medicine however sometimes it may be necessary to adjust your dose.

In terms of allergic reactions when using the cream, they are rare.  However, if you get any reactions to the cream, seek medical attention immediately.

One other thing that you need to note when taking bioidentical hormones is that over time, the estriol-estradiol compounds can lead to depletion of certain nutrients in the body as well as vitamin C.  Our pharmacists can inform you about nutritional supplements which can be beneficial during therapy.

How do I use Bi-est cream?

The transdermal bi-est cream is prescribed mainly because it has better absorption compared to other forms of the medication.

The cream should be applied to an exposed area to allow maximum absorption while at home.

For the best results, switch between areas of application now and then.

Bi-est cream should be applied to a clean area which is why it is recommended to apply it after a shower.  This also allows it to absorb better.

Place a small amount of the cream on the inside of your arm and leave for a few minutes.  Anywhere from five to thirty minutes should be good to let you know if your body will react negatively to the cream.

If you don’t experience any reactions such as irritation and redness, you can then proceed with the normal application.

Follow the directions of application as instructed by your doctor/prescriber.  Speak to your pharmacist or prescriber as soon as possible if you experience any kind of unexpected adverse reaction to the cream.

Bi-est creams are usually compounded to a specific dose depending on the patient requirements.

The ingredients are combined to form a smooth, paraben and fragrance-free cream that is hypoallergenic and non-greasy.

Apply the cream in the prescribed dosage on the chosen area and rub in for a minute or two to ensure that the cream is absorbed well into the skin.

Wash your hands with soap and water after applying the cream and avoid touching other people since the cream is easily absorbed through the skin.

Does Bi-est cream increase breast cancer risk?

The main concern with hormone prescriptions like Bi-est cream is whether such a drug can increase women’s risk of breast cancer.

According to various studies, estrogens have been shown to potentially increase the risk of endometrial cancer.  This is why a progesterone is often added to hormone therapy in order to reduce this risk.

However, there is no increased breast cancer risk when estrogen is used alone in hormone therapy for up to seven years. If you intend to be on the treatment for longer, however, talk to your prescriber about such risks.

Another study that was conducted on women who suffer from type 2 diabetes showed that there was improved insulin sensitivity after hormone replacement therapy due to the suppression of hepatic glucose that is produced by the body.

Other potential estrogen risks include ovarian cancer, drug induced pancreatitis, dementia, stroke, and blood clots.

Some of the risks involved, to a high extent, depend on how long you have been on treatment. This is why it’s very important that certain protocols are observed when treating low estrogen levels.  Such as medications being used at the lowest doses posdible for only the shortest period required.

One of the responsibilities of prescribers as well as pharmacists is to explain and guide you on the dosage as well as how long you need to be on the medication.

At the end of the day, a good bioidentical hormone replacement program should provide all the necessary information and education resources you will need.  The provider should also explain the potential risks and honor your patient rights while fulfilling any specific dosage request you may have as part of the process.
